Apollo HomeCare, Apollo AyurVAID collaborate to improve post-hospital recovery

India,  May 21: Apollo HomeCare and Apollo AyurVAID have come together to address the gap patients face between discharge and getting back home safely. To bridge this gap, the two have launched an integrated Transition Care service, offering patients access to a clinically monitored, in-facility step-down care environment where they are assessed, stabilised, and prepared for safe return home.

Addressing Recovery Gaps Beyond Hospitalisation: Apollo HomeCare and Apollo AyurVAID Partner to Strengthen Transition Care.

The Transition Care facilities are an intermediate care ecosystem located between the hospital and home. At the facility, patients continue to receive the care their condition still needs including nursing support, physiotherapy, rehabilitation, nutritional advice, and selective Ayurvedic therapies based on where they are in their recovery. The focus throughout is on one clear outcome: making sure the patient is stable, stronger, and genuinely ready to go home before they do. Doctors and care teams from both the partners work together on the ground, keeping a close watch on each patient’s progress and acting quickly if anything changes.

A key feature of this collaboration is the joint clinical approach. Patients enrolled through either of the healthcare service undergo a combined evaluation by specialists from both systems of care. This shared assessment drives a personalised transition care plan for each patient that integrates modern clinical medicine with selective Ayurvedic therapies where appropriate. The Transition Care service is currently operational in Bengaluru, Chennai, and Delhi with the availability of multiple beds.

The service is designed not only for patients based locally in these cities, but also for those who travel from other states and countries for major treatment procedures. This integrated care system provides a monitored step-down phase that allows these patients to stabilise fully before undertaking travel, reducing the clinical risks associated with early discharge and long-distance movement during recovery.

Apollo HomeCare’s experience in post-discharge clinical care combines with Apollo AyurVAID’s precision approach, NABH-accredited facilities, and established protocols for clinical documentation and outcome tracking. Together, they form a care partnership built around continuity rather than just handoff.

Commenting on the collaboration, Vishal Lathwal, CEO, Apollo Home Healthcare, said,

“Continuity of care does not end at hospital discharge. In fact, the transition phase that follows is where many avoidable complications and re-admissions originate. We believe in the promise of integrated medicine, bringing modern medicine and Ayurveda together through a structured transition care model.”
